Field Day Parade Entry Form

Want to be in the parade? Decorate your bike, tractor or car to ride in the parade? Enter a float with your family, friends or organization? March as an individual or as part of a group? Complete the form below to register.

Vehicles MUST be street legal and rating to tow a trailer, if applicable.

I will arrive at the EMS parking lot via Fieldcrest Drive, entering next to Homestead Plaza, between 10:15 - 10:30am to line up for the parade. I understand that all of Main St will be closed at 10:45am before the parade. All entries will be lined up on Petticoat Bridge Road from Columbus Road up to Fieldcrest Drive.
